Well, I don't think there's any problem posting this version (you want pp.28-30 of the main document, pp.40-42 in the pdf).

This is the earliest real world reference to it I could find, although I heard a while back that the original Traveller version in JTAS 11 was based on a proposed NASA design for either Skylab or the Space Shuttle.
