I don't know if these references help, but they're what I've garnered from a search of the 3rd edition of my Bibliography (with the publishers as we speak!  Yay!) and my Periodical Bibliography.  So bear in mind - particularly for the book bib rather than the periodical bib - that this only finds mentions in the contents or comments of the bibliographies - i.e. where I felt, at the time, the content merited a mention.  Unfortunately I've NOT indexed individual names of NPCs etc unless they struck me at the time as particularly notable, so I very much doubt this is in any way exhaustive.
