Do you have a current SSL certificate on your proxy server? I had issues towards the beginning of the year with this and other titles that are https:

Subject: [SERIALST] Remote Access issues : American Nurse Today
Dear Collective Wisdom,
Have any of you experienced and resolved remote access issues with the journal: American Nurse Today?
We are able to access articles to the journal on campus, however, off campus access remains “locked.”
We have checked to make sure that they have our IP range but nothing we have done has worked.
We are authenticating off campus users through EZ Proxy software and our remote servers fall within our IP ranges.
If anyone has had a problem with access and can give us some clues how to get ours to work we would be most appreciative.
The publisher has not been able to assist us in resolving this problem.
