You are invited to participate in a confidential survey on “The Balance of Authority and Responsibility,” a chapter that will appear in a book entitle Middle Management in Academic and Public Libraries.  Data collected from this confidential survey will be aggregated and used in the chapter.  Quotes from the comments, however, may be used anonymously.  Any potential references that might identify a participant will be deleted or anonymized.

You are invited to participate if you are or have been a middle manager.  You must be 18 years of age or older to participate.  There are no risks or benefits to you in participating in this survey. You may choose to participate or not. You may answer only the questions you feel comfortable answering, and you may stop at any time.  If you do not wish to participate, you may simply close the survey at any time, with no penalty to yourself.  If you do participate, completion and return of the survey indicates your consent to the above conditions.

You do not need to put your name on this form; however, there is an opportunity to participate further through a telephone interview, in which case, there are instructions for either inserting your name and contact information or, to keep your survey anonymous, emailing the principal investigator, Aline Soules, with your contact information.
