I've had a stainless steel Kleen Kanteen for several years. It's not insulated, but I have one of their insulated totes which works fairly well. Now they have many variations and accessories ...including an insulated version. http://www.kleankanteen.com/index.php

Mine has become the Kanteen Classic model. It fits fine in a bottle cage with the tote. Anything larger than 27 ounces will not fit. It is hard to drink from while actually riding ...because you can't squeeze steel. Because of that, I use mine mostly for loaded touring. But maybe this new cap will work better.

I don't recommend putting hot water in a plastic bottle - for health reasons.  For hot drinks, I have used insulated aluminum bottles made by hydro flask.  They keep your hot drinks really hot for several hours and are great for cold weather touring!  And they fit in your cage!

Also the Camelbak have a great insulated bottle, currently 20 oz are available, but the 25 oz should be in stores in the next couple of days.  Of course, they say Chill, but what keeps cold in also works with heat!

Polar bottles are insulated, so they might work.
