In the first block under Destinations Page (Selected Pages) click on Jersey Orders Due NLT September 1.

 

Read about the jersey if you want, then click on Jersey Order Form.  Place your order by adjusting the quantity for each item, and select the options that you prefer.  Be aware that if you use your mouse wheel, it will change the last thing that you adjusted if it was from a drop down menu; to avoid this, use your tab key to move between fields.

 

When you finish making your selections, click on the Produce Order Form button.  Check out the resulting form to make sure it says what you intended it to say.  If it doesn’t, simply close it and make adjustments on the order page, then produce another form.  When it is right, print it and send it in with your check.
