Excellent opportunity for an experienced professional! Barron County is accepting resumes for a full-time (40 hours/wk) Caregiver Specialist/Health Promotions Coordinator to provide information, linkage to resources, and assistance with access to services for older adults and their caregivers.  This position oversees our State Certified adult respite program Daybreak and coordinates Healthy Aging workshops for the multi-county region (Barron, Washburn and Rusk Counties). 

 

Must possess a Bachelor’s degree in Health or social services field and two (2) years work experience directly with people with chronic conditions, including dementia and family/informal caregivers (e.g. providing direct care, caregiver support, support group facilitation, residential care management, home care).  Community-based care experience required.  Must possess a valid driver’s license. 

 

Qualified candidates will possess motivational interviewing skills, with the ability to assess and reframe as the conversation progresses; knowledge of community resources and services available to persons with chronic health conditions and their caregivers; experience with family caregivers and family dynamics; knowledge of federal and state laws that regulate adult respite programs; and ability to assist individuals to coordinate effective personal planning and assist individuals in following through with the plan. Candidates must be proficient with public speaking and computer skills.

 

$24.69 - $32.35/hour with excellent benefits!
